HIPAA Notice of Privacy Practices

Our commitment

The Root Cause NP is committed to protecting the privacy of your protected health information ("PHI"). We are required by the Health Insurance Portability and Accountability Act ("HIPAA") to maintain the privacy of your PHI, to provide you with this Notice describing our legal duties and privacy practices with respect to your PHI, and to follow the terms of the Notice currently in effect.

How we may use and disclose your PHI

• Treatment. To provide, coordinate, or manage your psychiatric care, including consultations with other providers involved in your care.
• Payment. To obtain payment for services rendered — for example, to process credit-card transactions or provide superbills for out-of-network reimbursement.
• Health care operations. To support quality-of-care activities, licensing, credentialing, and business functions of the practice.
• As required by law. Including mandated reporting of suspected abuse, threats of harm, and court orders.
• With your written authorization. For any use or disclosure not otherwise described in this Notice.

Your rights regarding your PHI

• Access. The right to inspect and obtain a copy of your PHI, subject to certain limitations.
• Amendment. The right to request an amendment to PHI you believe is inaccurate or incomplete.
• Accounting of disclosures. The right to request a list of certain disclosures we have made of your PHI.
• Restrictions. The right to request restrictions on certain uses and disclosures of your PHI; we are not required to agree to all requested restrictions.
• Confidential communications. The right to request that we communicate with you in a specific way or at a specific location.
• Paper copy. The right to receive a paper copy of this Notice at any time.
• Breach notification. The right to be notified in the event of a breach of your unsecured PHI.

Changes to this Notice

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have about you as well as any information we receive in the future.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with the practice at the contact below, or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
